Ultrasounds are always considered “Diagnostic Imaging”.   LBP 4 will cover Ultrasounds performed on a pregnant uterus at 0% patient responsibility once per trimester.  All other imaging services will not be covered outside of this limit.  MVP has the same coverage as LBP 4, however any other ultrasounds done after the 1 per trimester limit will be covered based on your “Diagnostic Imaging” coverages on your SOB.
