What is considered a Major Medical Service?

Major Medical Services include surgeries, tests or other procedures that

are not immediately life threatening or not required for emergency services. 

Essentially, if it’s not a doctor’s visit, basic lab test (such as A1C, urine bacterial culture, etc.), or basic imaging (such as an X-Ray), then it’s a major medical service.  If there is a test or procedure that is needed during the 90-day waiting period, it may be approved for coverage with Prior Authorization.
